The Lambton HART (Homelessness and Addiction Recovery Treatment) Hub is a community-based network of services that helps individuals facing complex barriers in accessing care. Access includes support for primary care, housing, mental health and addiction treatment, and employment assistance. Through a "no wrong door" approach, individuals can connect to these services, including transitional recovery beds, a Drop-In Centre. Led by the Canadian Mental Health Association Lambton Kent, additional partners include the North Lambton Community Health Centre, County of Lambton, Bluewater Health, Sarnia-Lambton Ontario Health Team, Indwell, and Westover Treatment Centre.
